LIABILITY WAIVER & RELEASE AGREEMENT

I understand that participation in softball, baseball, strength training, conditioning, and related activities at The Diamond on 5 involves inherent risks. I voluntarily assume all risks, known or unknown.

I hereby release and hold harmless The Diamond on 5, its owners, employees, coaches, contractors, and volunteers from any and all liability for injury, disability, death, or property damage arising from participation or presence at the facility.

I agree to indemnify and hold harmless The Diamond on 5 from any claims brought by me, my family, my estate, or third parties related to my participation.

I authorize staff to provide or obtain emergency medical care if needed. I accept full responsibility for any resulting medical expenses.

I agree to follow all posted and communicated facility rules. Failure to comply may result in removal without refund.

I have read and understand this waiver. I voluntarily agree to its terms.

• A signed waiver is required before participation.
• No food, gum, or sunflower seeds on turf.
• Water/sports drinks only (no glass).
• Proper athletic attire required (no metal cleats).
• Respect staff, athletes, and equipment.
• Parents must remain in designated viewing areas.
• No horseplay or unsafe behavior.
• Report unsafe conditions immediately.

• Helmets required in all hitting areas.
• Only staff may adjust pitching machines.
• Wait for coach’s signal before entering cages.
• No swinging bats outside designated zones.
• Return equipment after use.
• Keep personal items out of walkways.

• Follow all staff instructions.
• No profanity, bullying, or harassment.
• Unsafe behavior may result in removal.
• Report injuries immediately.
• The facility is not responsible for lost or stolen items.

• Allow coaches to coach.
• Keep siblings supervised.
• Encourage positive behavior.

• Dispose of trash properly.
• Keep restrooms clean.
• Treat the facility with respect.

By entering The Diamond on 5, I acknowledge that I have read and agree to follow all facility rules.
